New Approaches for the Management of Hypertension and Cardiovascular Complications

Complications Hypertension plays a pivotal role in the development of a range of cardiovascular events in a progressive continuum of vascular damage. Hence, powerful blood pressure reduction and achievement of aggressive goals are important to protect against long-term cardiovascular morbidity and mortality in high-risk patients.

The recently published revised European Society of Hypertension/European Society of Cardiology guidelines will help broaden our approaches to the management of hypertension. The additional and unique benefits that some antihypertensives offer must be considered in treatment decisions: optimal characteristics include sustained 24-hour blood pressure control and renin-angiotensin system (RAS) inhibition.

In addition, albuminuria is a predictive tool for cardiovascular disease, and powerful blood pressure reduction using angiotensin receptor blockers (ARBs), as well as angiotensin-converting enzyme inhibitors, has been shown to slow the progression of microalbuminuria to macroproteinuria in hypertensive patients at cardiovascular risk. Furthermore, hypertension is perhaps the most controllable among the many modifiable risk factors for stroke and the potential pleiotropic effects of ARBs beyond blood pressure control may contribute to primary and secondary cerebrovascular protection in patients at risk.
